We used Ask Blue J to write a response:

Your registration status changed to "incomplete" because the IRS identified an issue with your registration. Several specific issues could have caused this change:

  1. Incomplete Required Fields: All required fields on the paper registration application were not completed.
  2. Unrecognized QI-EIN: The Foreign Financial Institution (FFI) identified itself as a Qualified Intermediary with a QI-EIN that the IRS has no record of.
  3. Improper Responsible Officer (RO) Identification: The RO was identified with initials only, or the RO’s full name was not provided. Additionally, the RO does not appear to be a natural person.
  4. Duplicate Registration: The registration application is a duplicate.
  5. Improper Certification: Part 4, the certification, was not properly completed. Part 4 must be completed by a natural person.
  6. Failure to Renew FFI Agreement: The FFI agreement was not renewed.

Additionally, if the registration was initiated or submitted but not completed within 28 days, the status would change to "incomplete." This applies both to registrations that were in the "initiated" or "registration submitted" status and those that were in the "approved" status but not completed within the specified timeframe.

To correct this, you need to log into your FATCA account, update the necessary information, and resubmit the registration. The system allows you to edit and complete the registration from the beginning of Part 1 or Part 2 of the online registration. Once the registration is updated and resubmitted, the status can change back to "registration submitted" or "approved."
